Resource Facilitation Service (RFS)

Through-out Montana there are many individuals and families who are dealing with the issues relating to a brain injury. The Brain Injury Association of Montana’ mission is to provide education and advocacy for these individuals. And they do for many in the state. But there are also many who have never heard of the Association or how we can help.The Resource Facilitation Service, recently implemented by BIAM, is an effort to reach out to these families and thrivers of a brain injury.

When facing the unknown after brain injury these individuals now have someplace to turn. By signing a release of information at the time of discharge from a hospital, emergency room or rehabilitation center the individuals name and phone number can be released to the Resource Facilitator of the Brain Injury Association. The Facilitator will contact the family or individual to begin the relationship which may last up to 2 years.The consumer driver program allows the individual to determine the level of support needed. The family or individual is welcome to contact their Resource Facilitator as questions arise or to discontinue services at anytime.

The benefits to the consumers are:

  • They will receive scheduled calls at regular intervals to help problem solve issues and identify resources that they need to succeed after brain injury.
  • The Resource Facilitator will help identify community resources unique to their region of the state. She will help the individual navigate the system.

The benefits to the professional:

  • This service will help support the individual and family after they leave the hospital to cope with the issues of living with a brain injury.
  • The service will save time and provide support to the discharge planners, nurses, social workers and other involved with the discharge process.

While this program is being implemented BIAM will be contacting many of the members who are currently in their system. Please contact BIAM to up-date your address and phone number or to request information and support.

This service does not replace any medical or rehabilitation follow-up that may be needed, but is intended to provide ongoing support for consumers and their families.
